Curcuminoids plus piperine improve nonalcoholic fatty liver disease: A clinical trial

Nonalcoholic fatty liver disease (NAFLD) is a most common liver disease condition that is prevalently seen in one out of every three people in general, one out of every two people with lipid disorders and in almost all the people with diabetes. This condition is associated with several chronic metabolic disorders including the increased risk of morbidity and mortality. Thus, lifestyle modification and metabolic control are must in NAFLD, and to date there are no US FDA-approved medications to NAFLD. Neutraceuticals viz. polyphenols have multidue of health benefits, may improve the liver health and be the choice of novel treatment option for NAFLD.

 

Objective:

To investigate the effects of curcumin co-administered with piperine on serum hepatic enzymes, lipid profile, and glycemic indices in patients with NAFLD.

 

Methodology:

Randomized controlled parallel-group trial: 70 adult subjects with ultrasound-determined NAFLD were randomized to receive either curcumin capsules (500 mg curcuminoids co-administered with 5 mg piperine) daily (n = 35) developed by Sami Labs Limited or identical placebo capsules (n = 35) after the meals for 12 weeks. All patients received dietary and lifestyle modification advises before the start of the study. NAFLD severity (on the basis of sonography) and liver function assessment at baseline and at the study end were performed.

 

Results and Discussion:

  • All the recruited seventy subjects completed the study
  • The administration of curcuminoids along with piperine significantly improved NAFLD severity compared to baseline (P<0.001) as well as compared to the placebo group (P=0.022)
  • Further, the percentage of improved patients was marginally higher in the curcuminoids plus piperine group compared to the placebo group (P=0.058)
  • Further, the supplementation of curcuminoids along with piperine in patients with NAFLD significantly reduced the
    • Lipid profile viz. cholesterol (P<0.016), low-density lipoprotein cholesterol (P<0.017)
    • Serum liver function markers viz. alanine aminotransferase (P=0.035), aspartate aminotransferase (P=0.042), and alkaline phosphatase (P=0.004)
  • Haematological parameters were not altered out of the normal range; suggesting no adverse effects with the curcumin plus piperine supplementation

 

Conclusion:

This study demonstrated the beneficial role of daily dietary supplementation of curcuminoids along with piperine to reduce the NAFLD disease severity and showed no adverse effects.
